The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 250+ ppm, rather than 50-100 ppm as required. A recipe was created with person in charge for dishwashers to follow. Post recipe above sanitizing compartment for proper sanitization of dishware. Provide photo/video of proper concentration.

Chartwells @ Owen J. Roberts High School, 981 Ridge Rd. Pottstown, April 26. Pass. At the high-temp dishwasher, a method for measuring the dish surface temperature of items washed in the unit was not initially being used. Dishwasher temperature 160°F test stickers were located on location. Employees were advised to use the test stickers daily to ensure the dish surface temperature is reaching at least 160°F. In walk in freezer #1, the insulation around the pipes behind the fan is not completely covering the pipes, which is allowing condensation and ice to form. Within a week, provide proof of repair (such as a picture or a copy of a work order) of the insulation showing that it has been repaired to be completely covering the pipes. At the hood over the steam jacketed kettle, where the upper section of the hood meets the wall, the caulking sealing the junction here is degrading and falling off the wall. Remove the old caulk and replace with new caulk. The following areas need to be cleaned: The floor beneath shelving in the dry storage area; The floor beneath shelving in the walk in coolers; The floor of walk in freezer #2 where ice is collecting. The ice is not falling onto any foods, but it is causing a slipping hazard for employees.

Cubed, cooked chicken was in a bowl sitting on top a pile of cooked chicken breast (that were over stocked). Chicken in bowl was 54°F. Chicken that was over stocked (up out of the pan) was 52°F. All chicken over 41°F was discarded. All foods must be stored in bain marie pans. Do not sit extra pans on top. Do not over fill pans. Keep extra in the bottom of the unit for quick restock. Aioli was not stored in refrigeration. Temp was 72-73°F. Aioli was reportedly set out of the bain marie this morning. Product was discarded. Aioli must be kept refrigerated or submerged in ice to maintain the product at 41°F or less. Grab and go juices must be date marked with date made. A date code system can be used but it must be documented and available for review during inspection. Sliced lunch meat was dated 4/17. These are past their 7 day use by date. Discard this product. Ensure foods are used within 7 days of date made/sliced – day made is day 1. Clean the following areas: Wall to right of oven; Panini machines and tables; Floor in back prep area – under equipment and sink including sink pipes; Replace soiled rag baskets.

The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Re-train staff. Some foods in the customer self-service areas were not properly labeled: packaged pizza slices in hot case; loose baked goods in the bakery case (eclairs, glazed donuts) Provide proper labels which include ingredients, sub-ingredients and allergens. Sani buckets need to be located at three locations through out the day, sushi prep, seafood prep, and cook line. Train all staff to complete this daily tasks upon arrival to work. Matt corrected on site with a bleach solution. Portioned Tarter and cocktail sauce in soufflé cups observed stored on shelf at room temperature food was held at 72°F, rather than 41°F or below as required. Maintain product in refrigeration or provide a letter from PDA (Pennsylvania Department of Agricultural) indicating these products are shelf stable. Discarded on site. This is a repeat violation, retrain staff. Discarded. A dredging station with flour needs to have a Time As Public Health Control Policy. Submit the attached CCHD TPHC Policy to the Department within 5 days. The flour needs to be discarded every 4 hours. Post literature at dredging station indicating such for staff to reference. Obtain three work sink stoppers for the three bay sink. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Obtain Chlorine test strips. Hand washing sink has been removed from the ware washing room. Cannot remove hand washing sinks without informing the Department. This is a major issue/problem. Notify plumber at once and install hand washing sink. This sink shall be equipped with a paper towel dispenser, soap dispenser, and trash can. The light intensity in the food preparation area is not at least 50 foot candles. Light fixture removed over sushi prep area; install missing light above hand washing and food prep sink. Rear hall way corridor/dry storage area: no lights operable in this area. Facility was completely renovated about 4 years ago. Facility is showing signs of significant ware and tear. Repair the following: Missing base behind dry storage shelf and microwave table next to ware washing room; Wall observed deteriorated at wall/floor juncture located behind walk in cooler door, install new cove base; General paint is peeling in areas in back hall way where dry storage may occur, located at wall/floor juncture; Exposed drywall and wood on wall behind ware washing sink, seal up/cap this corner wall edge.

Mochi Sushi Inc., 668 Lancaster Ave. Berwyn, Opening, April 25. Pass. Temperature measuring device for ensuring proper temperature of equipment is not available in the 2-door refrigerator in the kitchen. Acquire thermometer immediately. Observed the ice machine, a food contact surface, to have a buildup of biofilm on the interior. Clean and sanitize the interior of the machine to prevent potential contamination of ice. Clean debris from the 3 chest freezers in the basement. Gender neutral restroom did not have a waste receptacle with a lid for feminine hygiene products. Provide immediately. Observed LED lights to be burnt out in above the kitchen prep area. Replace lights immediately for adequate visibility. Remove dead rodent from trap in the basement. No active pest problem was observed at facility.

Pickering Valley Golf Club, 450 S. White Horse Rd. Phoenixville, April 25. Fail. Hot dogs in the hot dog warmer were at 113°F, rather than 135°F or above as required. Hot dogs were discarded. Check the hot dog temperature at least once an hour with a probe thermometer to make sure that they are at least 135°F. The large 3-compartment sink in the area behind the bar, and the upstairs kitchen 3-compartment sink both need quaternary ammonia test strips kept nearby, so that staff can ensure the sanitizer is between 200-400 ppm. Provide proof of purchase (such as a picture or receipt) of quat test strips within a week. Quat test strips are also called QT-10 test strips. At the bar 3-compartment sink, the quaternary ammonia sanitizer was initially too low to measure. Additional tablets were added, and the final sanitizer concentration was at 200 ppm. Test sanitizing solutions every day with the sanitizer test strips, to ensure the sanitizer is between 200-400 ppm. In the area behind the bar kitchen, the duct next to the ice machine is leaking and creating a puddle on the floor. Repair the leak by 5/2/2024. Rodent droppings were observed in the area behind the kitchen, around and behind the water heater. Clean and sanitize the area around and behind the water heater, clean any new droppings as soon as they are found, and have a licensed pest control operator service the facility, with extra attention to areas where droppings were found. A can of Raid insect killer was observed stored in the kitchen back storage area. Raid is a restricted use pesticide not approved for use in a food facility, and it cannot be used or stored in food areas. The can of Raid was removed.

It was indicated by person in charge that a technical error had occurred with the electronic date marking system. This was corrected while on-site. Maintain foods properly date marked. Repair the following: Leak at the drain of the handwashing sink next to the 3-compartment sink; Leak at drain of the 3-compartment sink; Leak at the drain of the ice machine; Repair ice machine to prevent dripping of water from the upper edge of the lid. Clean the following: Floor under shelves behind the grab and go beverage refrigerator; Floor around the ice machine. Regrout between the floor tiles in front of the ice machine where it is low and water/food debris are accumulating. Use an epoxy based or similar grout designed for food facility. The 2-door refrigerator is not in use. Repair or remove from facility. Maintain facility free of any unnecessary equipment.

Observed ambient temperature of the under counter glassfront refrigerator at 54°F. The thermometer inside this unit was observed at 54°F. The following internal food temperatures were observed: milk 49°F; half & half 53°F; almond milk 52°F; whip cream 50°F; oat milk 44°F. All TCS foods (dairy) >41°F were discarded during this inspection. Do not use this unit to store TCS foods until it has been serviced and is operating at 41°F or less. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the Avancto bain marie, is not being date marked. TCS foods prepared in the facility were date marked during the inspection. Ensure consistent date marking of all ready to eat TCS foods. Person in charge reheated to required temperatures and put back into hot holding. Commercially processed ready to eat food, deli meats, located in the walk-in refrigerator, and held more than 24 hours, are not being marked with the date it was opened. Provide date marks and follow the 1-week date marking protocol for food safety to prevent pathogen growth. Time/Temperature Controlled for Safety (TCS) ready-to-eat deli meats repackaged by the food facility, were being datemarked and were beyond 1-week date. Deli meats were discarded by person in charge to prevent potential pathogen growth. Time as a public health control for fresh pizza slices being used at facility. Facility has a time log for when to discard slices after 4 hours, however, facility has not kept up with the log. Ensure to keep up with the log to discard at proper times and prevent potential pathogen growth. In the dishwashing area, observed multiple floor tiles in disrepair with holes and exposed wood beneath tiles. Floor must be constructed with smooth, non-absorbent, and easily cleanable materials. Replace floor tiles within given timeframe. Thermometers for ensuring proper temperatures of food were not available. Probe thermometer was acquired by person in charge later during the inspection. Ensure to calibrate and utilize. Observed the ice machine, a food contact surface, to have a buildup of bio-growth on the interior ice chute. Ensure to clean and sanitize so it is clean to sight and touch before using ice again. Clean the following: Hood filters of excess grease; Shelves in walk-in refrigerator of mold build-up.

To use time as a health control for food rather than temperature until the unit is repaired, a time log must be started indicating when foods or trays of food are removed from cold holding, and foods must be used or discarded within 4 hours. At both kitchen hand sinks, the caulking behind the sinks is degrading, which could allow moisture to begin collecting behind the sinks. Reseal sinks to the wall. At the high-temperature dishwasher, the final dish surface temperature was at 147.9°F, rather than 160°F or higher as required to sanitize dishes. Within 10 days, dish machine must be repaired to have a final rinse temperature between 180°F-195°F per manufacturer’s specifications. Until dish machine is repaired, any dishes run through the machine will be sanitized separately in the 3-compartment sink. Floor underneath all kitchen equipment, especially cook line equipment, has a buildup of food debris that is attracting pests. All floors under kitchen equipment must be thoroughly cleaned and sanitized. Floors must continue to be cleaned regularly so as to not attract pests. Rodent droppings were observed in the storage room with disposable single-use cups and utensils, and behind equipment in the cook line, especially in the far corner at the floor/wall junction. To address this: Thoroughly clean the entire floor, especially behind and under equipment and at the floor/wall junction; Maintain the floor in clean condition, especially underneath equipment; Clean any new droppings as soon as they are found; Contact the pest control company to have them service the kitchen, with extra attention paid to the area behind the cook line, and any areas where new droppings are observed.

Replace three (3) grey plastic bins used to store tortilla chips-they are very greasy inside and out and the outside of the bins have some black film. Be certain to replace with food grade containers. Correct within 7 days. CHICKEN PREP: Clean the following: The faucet handles to ALL sinks; The exhaust hood canopy and filters; The wall under the hood and the floor under the hood. BAKERY: Clean the cooking exhaust hood. Correct within 7 days. PRODUCE: Observed clean food equipment air drying on the wash drain board of the 3 bay sink-also a pickle bucket was observed on this drain board as well and is being used to hang utensils and store wet utensils inside and not allowing time for draining and/or air-drying. Obtain a cart with shelves with holes for proper air drying or install shelving to air dry items or air dry items in shifts on the proper drain board-to the right of the sanitize sink. The clean drain board was full of items air drying as well. Correct today. CHICKEN: The handwash sink does not have water any hot running water-hot water must be supplied at a temperature of at least 85°F, correct immediately. CHICKEN: Prep Sink against half wall-repair the stripped hot water faucet. Prep Sink next to hood-supply cold running water. Correct within 48 hours. CHICKEN: Soap was not available at the hand wash sink next to the hood. Correct immediately.

Raw eggs were relocated below ready to eat foods during the inspection. Observed canned goods stored directly on the floor in the kitchen rather than 6 inches off of the floor as required. Move canned goods to be 6 inches off of the floor as required. Observed dirty food equipment stored in both the wash and rinse compartment. Observed dish soap stored inside the rinse compartment. Observed hand soap stored at the 3 compartment sink. Soaps were removed during the inspection. Use the wash compartment to store unclean equipment. Re-train staff. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration used in spray bottles. Obtain chlorine test strips. Observed clean food equipment stacked while wet and not allowing time for draining and/or air-drying. Re-train staff. The handwashing sink in the kitchen had a significant leak and was unusable. This is the only handwashing sink for the kitchen and must be available at all times or food service must stop. Leak was repaired during the inspection. The handwash sink in the kitchen was storing employee personal items and not accessible at all times for employee use. Employee personal items were removed during the inspection. Create a designated location for employee personal items. The handwashing sink is for handwashing ONLY. Relocate paper towel dispenser to be directly above the handwashing sink. Paper towel dispenser empty at the time of inspection. Napkins were received from the dining area. Supply dispenser with paper towels. Wall under the fire extinguisher in the kitchen has a hole and in need of repair. Repair hole and finish wall to be smooth and non-absorbent. Working container in kitchen area, used for storing a disinfectant taken from bulk supplies, was not marked with the common name of the chemical. Disinfectant was stored inside a reused Totally Awesome bottle and incorrectly labeled with “bleach”. Disinfectant was removed during the inspection. Disinfectant was being used on table tops. Disinfectant is NOT approved for this use. Disinfectant was replaced with an approved sanitizer during the inspection. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer.

During the inspection a staff member brought over new test strips. Ensure that you are checking the expiration dates on all sanitizer test strips frequently. Clean the following: Both microwaves; The entire mop sink room. The hot water at the three-compartment sink was at 104°F at the beginning of the inspection. A staff member fixed the hot water heater and the three compartment sink had a hot water temperature of 110°F. Ensure to monitor the hot water at all sinks and ensure that they are at proper temperatures at all times. Outside waste handling unit had drain plug removed. Obtain a new drain plug. In the dishwashing room, there is paint peeling on the walls. Pictures were taken by Octorara high school staff member. Resurface and seal the areas so that they are smooth, non-absorbent, and easily cleanable. Repair both of the broken light shields in the walk in cooler. Replace the bulb that is burned out in the walk in cooler. Fix the light in the mop sink room. In the dry storage area there is numerous ceiling tiles with a water stain. Fix any leaks and replace the ceiling tiles. Remove an unused/unnecessary equipment from the kitchen and serving area.

Repair the leak immediately. The school has plans to repair the walk in freezer in the summer when school is not in session to fix the ice build up. Clean the walk in cooler fan covers. The food-contact surfaces of equipment and utensils did not reach 160°F during the sanitizing cycle of the high temperature dishwasher as evidenced by the failure of the temperature indicator. The facility must manually sanitize all dishes in the three compartment sink. Plumbing system not maintained in good repair – observed the drain leaking at the three compartment sink onto the floor.

Garlic/oil mixture was voluntarily discarded. Observed in-use knives stored between cold holding units, an area not easily cleanable & sanitized. Knives were relocated during time of inspection. Ensure all utensils are stored in a location that can be cleaned and sanitized. Food utensils in cook line area observed stored in a container of water which is not maintained at 135°F. Water was measured at 110°F. Container was reheated during the inspection. Chicken and uncooked bacon were held at 50°F, in the walk-in cooler, rather than 41°F or below as required. Person in charge started that foods were from the food truck. Chicken and bacon were discarded during the inspection. Observed a non-food grade brush with plastic attached being stored inside a garlic/oil solution. Brush was discarded during the solution. Obtain food grade brushes. Condensing unit in pizza prep top observed leaking. Repair leaks. Person in charge stated part is ordered. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean under and behind the soda machine. The handwash sink in the back prep area was blocked by a large piece of equipment stored inside the sink and not accessible at all times for employee use. Equipment was removed during the inspection. Observed one insect control devices located in back prep area with potential to contaminate food, equipment, and/or utensils. Remove or relocate device to a non-food prep/storage area. Ceiling tiles missing above the walk-in cooler. Replace ceiling tiles.

King’s Garden Restaurant, 308 S. High St. West Chester, April 22. Fail. The Food Facility Operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. During the inspection the manager arrived on site. Ensure a Person in Charge is at the facility at all times. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. The Person in Charge needs to take a food handler class and if found at the follow up inspection that the person in charge did not retrain the employees on the FDA food code all of the employees will be required to take a food handler course. An open employee’s drink was observed in the kitchen. Employee drinks must have a lid and straw and must be stored away from food preparation and storage areas. Food was being stored in grocery bags in the coolers and freezers. These bags are not made for food storage. Food grade bags must be used. Replace grocery bags with food grade bags or food grade storage containers. The is a repeat violation. Several raw animal foods were stored incorrectly in the walk in cooler. Raw animal foods must be stored by the temperature at which they are cooked. This is a repeat violation. Food ingredient storage containers, throughout the kitchen, is not labeled with the common name of the food. Label all food ingredient storage containers with the common name of the food. The rice paddle was being stored in the rice warmer. Remove the rice paddle and store outside of the rice warmer. Observed wet wiping cloths in kitchen, not being stored in sanitizer solution. Wiping cloths for food contact surfaces must be stored in a container of sanitizer solution when not in use. Cooked rice, was not cooled from 135°F to 70°F in 2 hours and/or from 135°F to 41°F within 6 hours, after preparation. The facility cooked rice the day before. The rice was found in the walk in cooler with an internal temperature of 48°F. The rice was discarded on site. Observed food being cooled in large stock pots in walk-in cooler, which is not a proper cooling method. Retrain employees on proper cooling methods. Raw chicken was found on the counter at 48°F. The facility stated that they just placed it on the counter 5 minutes ago from the walk in cooler. The facility placed the raw chicken back into the walk in cooler. At two hours the raw chicken was checked and still had an internal temperature of 48°F. The raw chicken was in a huge pot. The facility needs to ensure that when they are prepping chicken they are prepping a small amount at a time and not leaving the chicken out for long periods of time. The walk in cooler had an ambient temperature of 39 degrees and other food items found stored in the walk in had an internal temperature of 41 degrees or below. The chicken was discarded. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the walk in refrigerator, the food prep table and the 1 Door Refrigerator is not being date marked. Must date all food items with the date they are made. Correct today and comply everyday. EHS observed the facility using containers that are not meant to store food items in. Purchase food grade containers to store food items in. Food thermometer is not scaled to measure to +/- 2°F. The facility must obtain a new thermometer or calibrate their thermometer. Food facility does not have a mechanical clothes washer and dryer for linens laundered on the premises. Dirty rags were observed air drying on a wire in the kitchen. Rags must be laundered on site with a mechanical clothes washer and dryer or must be sent out to a company to be properly washed. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. The test strips on site expired in 2011. The facility needs to obtain new chlorine test strips. Employee observed just rinsing a bowl in the food preparation sink and not manually washing the bowl in the three compartment sink to wash, rinse, and sanitize. During the inspection the employee was told to wash, rinse, and sanitize the equipment. Facility must thoroughly wash, rinse, and sanitize all food contact surfaces. The following must be completed: Remove all white plastic film from the lid of the food prep table; Clean all floors specifically under the tables; Clean the interior and exterior of the refrigerators; The facility needs to organize and clean the basement, empty pizza boxes were observed throughout the basement; Thoroughly clean the employee restroom; Remove any cardboard or paper that is lining the shelves; Clean and sanitize all of the food storage containers. The food preparation sink does not have an adequate air gap of at least 1 inch, between the water supply and flood rim of the sink. Install an air gap in the food preparation sink. The grease trap is not connected and the observed plastic bags to be shoved into the hole. An odor is coming from the grease trap as well. Facility must have the grease trap inspected by a professional and cleaned out. Observed cardboard being utilized as a floor covering in the basement. Remove the cardboard that is being utilized as floor covering. The floor/wall juncture throughout the entire kitchen area is not coved and closed to 1/32 inch. Inspect the entire facility and repair any broken cove bases. Facility needs to inspect the entire facility and repair any holes in the walls or ceilings. Broken equipment, pallets stored in the outside area of the food facility needs removed. Remove any trash, broken equipment or pallets that are stored outside and clean up the area. Old unused equipment stored throughout the kitchen area, should be removed from food facility. A chlorine sanitizer that was not for food contact surfaces was being used in the 3-compartment sink. A new bottle of chlorine sanitizer was purchased. The 3-compartment sink was set up with the new chlorine sanitizer to the correct concentration.

Speedway #06734, 108 N. Pottstown Pike, Exton, April 22. Fail. Milk was held at 52°F, in the coffee area, rather than 41°F or below as required. Milk was discarded during the inspection. Unit cannot be used until it can maintain foods at 41°F or below as required. Milk, a potentially hazardous ready to eat food requiring datemarking, was beyond the date-marking and required discarding. Milk dated 4/19 was discarded during the inspection. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following: Floors under the 3 compartment sink, under the soda boxes, and under the interceptor; Counter under the Big Gulp machine; Counter behind the bun warmer. Ceiling tiles stained above the “Soft Drinks” cooler section and in the warweashing area. Ensure there are no leaks and replace the ceiling tiles. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer. Move Certified Food Manager Certificate and License to Operate to be visible to the public.

Time in lieu of temperature being used in the food facility to control ready to eat Time/Temperature Controlled for Safety (TCS) foods without written procedures or documentation to verify disposition of food. Observed fried egg rolls stored at room temperature on top of the chest freezer. It was reported that the egg rolls are partially cooked, placed into the refrigerator and then fried to 165°F prior to order. Time-tracking was initiated during this inspection. Ensure that the fried egg rolls are consumed or discarded within 4 hours.
